Guidance for Professional Staff working within Integrated Care Teams

HSC Trusts in Northern Ireland have established a number of Integrated Care Teams (ICTs), providing services to the population across the Trust wide area. Each ICT includes a range of health and social care staff who work together to offer patients a better quality service, with easier access to the services required. Treatment and services are provided, for example, through District Nursing, Occupational Therapy, Care Management and Social Care staff.  Nursing services are available 24 hours a day, 7 days a week, and include a rapid response service for GPs and Emergency Departments.

During 2015, NIPEC and the Northern Ireland Social Care Council (NISCC) agreed to work in partnership to explore the needs of professional staff managing and/or working within Integrated Care Teams across Northern Ireland.

A survey will be conducted with key stakeholders during February 2016 to ascertain the structure of ICTs within each of the five HSC Trusts and obtain an outline of requirements/needs and guidance, support and resources currently available for managers and professional staff working in ICTs as well as suggested areas for development/improvement. The findings of the survey will inform the development of a Project Initiation Document, including aims, objectives and deliverables. It is anticipated that further work to meet the aims and objectives will be taken forward from July 2016.
